Student/Application Information: Student/Parent/Guardian complete all information in PEN.
Pledge of Employer: Must be completed by employer: please make sure ALL information is completed. Employers cannot leave items marked 1-2-3-4 blank they can estimate (for regulatory purposes only)
Physician’s Certification for Minor work Permit (PAGE 2) must be completed by Physician. If the student has a current (within one year) athletic physical on file with the Athletic Office, Page 2 will not need to be completed.
Once all three sections are completed, please contact Mrs. Suto, Secretary, at Ravenna High School at gail.suto@ravennaschools.us for an appointment to complete your work permit.
The student MUST be present when obtaining the work permit as he/she must sign the printed work permit.
