There are action plans below for students with Asthma, Seizures and/or Severe allergies. Action plans need both parent and physician signatures to keep emergency medications at school. 

 

Under medication administration, the prescription form is for students needing to take a medication at school that is prescribed by their physician. It needs the parent and physician signatures. The non-prescription form is for students needing to take over-the-counter medications at school (Advil, Tylenol, etc.). This form only needs a parent signature.
