Preschool Registration

 

Preschool is registering new students electronically through FINAL FORMS, an online forms and data management service. For further details please see the parent instructions.

The first step in the registering your child for Preschool is to complete all Final Forms. Once finished, our office will contact you to schedule a screening appointment. All students must be screened before enrolling in Preschool.

The following items are needed to fully register your child for the Ravenna Preschool:

  • Driver’s license or photo ID
  • Proof of Residency- (Preschool students must be residents of the Ravenna School District)
      1. Your rental or purchase agreement or mortgage statement
        AND
      2. Your most recent utility bill (phone, gas or electric)
        (We CANNOT accept driver’s license, check, credit cards or regular mail)
  • Your child’s Birth Certificate
  • Your child’s Social Security number
  • Your child’s Immunization record
  • A completed physical from a doctor
  • Custody Papers (if applicable) – Must be court stamped, signed and state the name of the residential/custodial parent.
  • Proof of income for previous year- for Preschool enrollment only

All Documents can be uploaded to your Final Forms account.
How do I upload a document?

Log in, and go to your “My Students” page.
Click the “folder icon” in your student’s row.
Use the popup box to upload a document.

    Enrollment will take place AFTER the Final Forms and screening process has been completed.
