Central Registration is now registering new students in grades Kindergarten through 12th grade electronically through FINAL FORMS, an online forms and data management service.

 Ravenna School District K-12 Registration

To register your child for the upcoming school year.

• You may register your child electronically through FINAL FORMS, an online forms and data management service. You will have to upload documents to complete the registration. All sections of the online registration application and required documents must be complete and submitted for the application to be processed, and your child enrolled.
• Please follow these easy steps for online registration:

  2. Once you are logged into FINAL FORMS, please follow the steps and upload any documents listed below to complete your online registration. For further details see the parent instructions.

     1. Log in, and go to your My Students page.
     2. Click (Folder) in your student’s row.

• The following Registration Requirements are needed to fully register your child:
• NOTE: Upon initiating the online registration process, you will need to have all required documents accessible and ready to be scanned.
• 1. Proof of Residency (The residency documents must be in the name of the parent/guardian) Please provide BOTH
     Your rental or purchase agreement or mortgage statement
     AND
     Your most recent utility bill (gas or electric)
     (We CANNOT accept driver’s license, check, credit cards or regular mail for proof of residency)
  2. Your child’s Birth Certificate/Birth Record
  3. Your child’s Immunization Record
     
  4. Parent’s picture ID
  5. Complete Custody Papers (if applicable)
     1. All pages are required
     2. Must have a court stamp
     3. Must state the residential parent
     4. Must include the signature page

To register a child in kindergarten, the child  must be five years old on or before August 1.

Change of Address

If your family is moving within the Ravenna School District, the parent/guardian must complete a change of address for all students in the household.  The current lease/mortgage AND a utility bill must be provided in addition to updating FINAL FORMS with the new information.  The residency documents must be in the name of the parent/guardian.  If they are not, please contact the Central Registration office.

If you have moved out of the district, please follow the procedures above in addition to submitting an Open Enrollment form.
